Late reports of injury and illness can subject your department to the cost of state fines, and can be a detriment to effective claim handling.

Rapid reporting is absolutely key to assuring good medical recovery, and as shown in the charts below, is key to reducing lost days, overall costs and litigation.

Only a few days can make a significant difference.

Data Source: All Campus Indemnity Claims closed in Fiscal Year 1999.

PLEASE:

1. Instruct your employees to immediately report all injuries.
2. Within 24 hours: fill out the Supervisor Incident Report (SIR) form.
3. Fax the SIR form to Disability Management Services at Fax (415) 476-2328.
