Generally, you can change your medical group and/or PCP throughout the year. If you contact Health Net and request the change by the 15th of a month, the change will be effective the first of the following month.
The information below outlines the actions Health Net took prior to 1/1/10 to assist employees and retirees affected by the UCSF Medical Group transition to Hill Physicians medical group.
If you have a Brown and Toland PCP that is part of the UCSF Medical Group, Health Net will automatically transfer you and/or your family members to the same UCSF PCP with Hill Physicians effective January 1, 2010 You will receive a letter in mid-October outlining this information as well as information on transition of care.
If you would rather stay with Brown & Toland and not follow your PCP to Hill Physicians, you need to contact Health Net prior to December 15th, to stop the transfer and to select a new primary care provider for an effective date of January 1, 2010, (1-800-522-0088).
If you are a Health Net Member in Brown and Toland with a non-UCSF PCP, you/your family members will remain with your current PCP(s) in Brown & Toland. If you prefer to transition to Hill Physicians, contact Health Net customer service to switch to a new PCP in Hill Physicians, (1-800-522-0088).
