The Resume Supplemental: Obtaining Additional Information from Your Candidates
As a tool in our ongoing efforts to support due diligence and consistency in our hiring practices, the Resume Supplement is a required form that hiring managers provide to final applicants to obtain relevant information such as employment history and conviction disclosure. Two changes have been made to the conviction disclosure and social security sections of the form. The fist involves clarification on what types of offenses do not require an applicant to make a disclosure. Examples of offenses that may be omitted include traffic violations for which the fine imposed was $300.00 or less and convictions that have been sealed, expunged or legally eradicated.
The second point of clarification involves the removal of the section for disclosing an applicant’s social security number. There is no legal requirement that the University obtain an applicant’s social security number on a form of this type. Once a final applicant is hired, disclosure of this information will be required on the I-9 form.

Obtaining Information about Your Health Plan Benefits - Essential Resources for All Faculty and Staff
If you are looking for more information about your health plan benefits or wondering if a specific procedure or service is covered, the first place to start is the Evidence of Coverage Book specific to your health plan. They are all available on the At Your Service Website:
(Follow the link to the specific plan you are interested in and then select Evidence of Coverage Books from the Quick Link menu on the right hand side of the page.)
The EOC provides a summary of benefits; what will and will not be covered, your costs for services, other important plan information, and it should be able to provide answers to many questions about your benefits. These books are updated annually. Please always refer to the most recent version as your benefits may have changed from previous years. If you access them electronically, they are in an easily searchable PDF format which should allow you to obtain the specific information you need without reading the entire EOC.
You should also receive a paper copy of the plan EOC at the beginning of each year. Discard any outdated versions you may have unless you have pending claims from a previous year and may still need to refer to the EOC from that year. If you did not receive a printed version, or have misplaced the one you were sent, you can obtain a printed copy by calling the customer service number for your plan.
Still have questions ... Additional information about plan coverage and benefits may be obtained by calling Customer Service for your plan. Prepare a list of questions before you call and take notes on the conversation you have with the Customer Service Representative, including the date you called and the name of the individual you spoke with. If you feel the person you are speaking with does not understand your questions, ask to speak with a supervisor.
