New, More Detailed Conviction History Section of the Resume Supplement
The University utilizes the Resume Supplement form in order to gather relevant information regarding an applicant’s previous experience, education, and criminal conviction history prior to making an offer of employment. The University has revised the Resume Supplement form so that the conviction history section is now a separate document; however it remains a required component of the Resume Supplement. The revised conviction history form provides clarification as to which arrest and conviction information need not be disclosed by an applicant and includes a frequently asked questions section. Effectively immediately, please use this form for all final applicants that are being considered for a position with the University.

Disability Management Services: Disability Management Services coordinates the Workers’ Compensation program and provides support to employees with health problems and comprehensive consultation to supervisors and department managers. Working closely with internal and external partners, our services are designed to provide counseling and support to employees with disability issues, facilitate communication, minimize work disruption, and protect the health and productivity of our workforce.
