Final Reminder: Family Member Verification Deadline is May 17
May 17 is the deadline to submit your documents to verify your enrolled family members in UC sponsored benefits. To ensure you meet the deadline, please fax (866-412-0214) your documents or upload them directly into Secova’s secure database.
Employees that do not respond by May 17 risk losing all UC Benefits along with de-enrollment of their family members. This includes: medical, dental, vision, legal, supplemental life insurance, supplemental disability, and AD&D.
Certain faculty and staff do not need to participate in the verification process at this time including those who:
- Have no family members enrolled in UC-sponsored health insurance
- Post-docs
- Medical resident or House staff
- Students
If you did not receive a packet or if you have questions, call Secova at 877-632-8126. If your name is not in the Secova database, then you are not included in this phase of the review.
