New University of California Demographic Data Transmittal Form (U5605)
Effective August 1, 2010, the University of California is required to change the manner in which the race and ethnicity data of employees are collected and reported to the Integrated Postsecondary Education Data System (IPEDS). The changes result from the implementation of new standards for Federal agencies on the collection of racial and ethnic data adopted by the U.S. Department of Education in 2007.
As a result of the new requirements, UC personnel should provide to employees hired on or after August 1, 2010 the attached new Demographic Data Transmittal Form (Form U5605) to voluntarily self-identify their race, ethnicity, disabled status, and veteran status. The revised form will be available for ordering through KP Corporation and for download under the forms section of At Your Service (AYS).
Systems modifications have been completed and were distributed to all campuses via payroll release in early July 2010. The systems that have been updated are:
- Payroll Personnel Systems (PPS)
- Employee Data Base (EDB)
- Corporate Personnel System (CPS)
- Corporate Data Warehouse (CDW)
- Web New Hire
The IPEDS data submission window for the human resources survey in which these changes are required opens in early December 2010 and closes mid January 2011. Data available as of November 2011 is required and will be used for the submission of this survey.

Offered as either classroom-based or online, this class will help you participate effectively in the entire performance appraisal process.Instructor-led sessions are being offered on August 4, September 1, and October 6 at our Laurel Heights location from 9:00 a.m. to 12 p.m.
- Toolkit: Preparing for Your Performance Appraisal (online)
This toolkit will help you assess your readiness, write a self assessment, and keep a performance log.
To enroll: Log in to the UC Learning Center with your 9-digit UCSF employee ID. Search for each class title, click “register” to make your selection, and then click “submit” to register.

Managing Your Career
There is still time to enroll in our very popular Career Planning Workshop series. Past participants have said, “Thank you so much for an excellent and inspiring workshop. I really learned a lot.” And: “I will benefit from this training for my future career.”
These workshops were developed to encourage and enable employees to take charge of their own careers. There are four modules, and they can each be taken independently. However, the fourth and last module requires that participants attend the first three. We also encourage employees to attend all the modules to gain the greatest benefit.
