The StayWell Program is free and available to eligible faculty and staff and their family members age 18+ enrolled in a UC sponsored medical plan and living in the U.S. with the exception of Kaiser members (Kaiser members continue to have access to Kaiser’s fully integrated wellness resources).
StayWell Benefits Include:

We are currently developing our own Wellness Program at UCSF that will serve to enhance the wellness resources already offered at the University and will be available to ‘all’ faculty/staff. To kick-off our event we plan to start a ‘Walking Program’! We hope the ‘Walking Program’ will serve as a fun way for employees to enhance their health and to meet like minded people - more news on this to come in the following months.

Reminder About the Personnel Data Form
The Personnel Data Form is used when employees are hired or report a change of address. Some time ago, the University negotiated a change in this form with three unions: CUE, AFSCME and UPTE. Specifically, the language on the form related to the release of an employee’s home address was modified. For employees in bargaining units represented by these three unions, departments should utilize the UPAY 544A form. For all other employees, departments should utilize the UPAY 544 form. Both forms can be found on the Office of the President’s website at http://www.ucop.edu/payroll/forms.htm.

Health Care Reimbursement Account (HCRA) Spending Card Update
Spending Cards Are Good for Three Years
If you are enrolled in the UC Health Care Reimbursement Account (HCRA) program and use a SHPS spending account card, you should know that cards issued to you and any family members can be used for three consecutive calendar years after issuance—provided that you are enrolled in HCRA each year. You must re-enroll in HCRA each year during Open Enrollment to have an account in the coming year. Here is what this means to you:
- If you enrolled in HCRA in 2007 and received a card which you activated:
- Assuming you re-enrolled in HCRA for 2008, you may continue to use your card now and during the rest of 2008 as long as you have funds in your account - you will not receive a new card this year
- Your new 2008 annual HCRA spending amount has been loaded onto the card you received last year, and you do not need to reactivate it to use it.
- If you enrolled in HCRA for the first time for calendar year 2008:
- YOU SHOULD RECEIVE YOUR SPENDING ACCOUNT CARD BY JANUARY 20, 2008.
- Your card is ready for use as soon as you activate it, as the instructions received with your card explain.
- If you re-enroll in HCRA for 2009, you will continue to use the same card in 2009.
HCRA 2007 Grace Period
Those employees enrolled in HCRA that still have money in their accounts from 2007 have until March 15, 2008 to incur eligible expenses and until June 15, 2008, to submit claims and required documentation to SHPS to request reimbursement. These dates are based on IRS guidelines and are strictly adhered to, so once again please make note of these dates:
- March 15, 2008, to incur eligible expenses
- June 15, 2008, to submit claims for reimbursement
For more information please visit the ’At Your Service’, SHPS page.
