What Should You Do When a Work-Related Injury Occurs?

An employee may report an injury on the workers compensation hotline at 1-877-682-7778 or by completing and submitting an Employee Incident Report (EIR) to the DMS office by faxing it to: (415) 476-2328. The EIR can be downloaded at: http://ucsfhr.ucsf.edu/files/EIR.pdf

If you observe an accident, instruct your employee to immediately report all injuries; refer the employee to UCSF’s Designated Medical Provider; remove, tag, and store any broken equipment that caused the injury; and complete the Supervisor Incident Report (SIR) form and fax it to DMS within 24 hours. For additional information, please refer to the Supervisor incident protocol.
