Benefits: Family Member Verification Deadline is May 17
If you have dependents enrolled in UC sponsored benefits for faculty/staff and have not yet complied with the Family Member Verification (FMV) process, please take steps to do so now to ensure your benefits are not impacted! If you haven’t received or can’t find your personalized FMV packet, contact Secova at 877-632-8126, 24 hours a day, seven days a week to request your packet; services are available in multiple languages.
Employees with enrolled dependents have until May 17 to provide the required documentation to Secova, the vendor facilitating this audit. Employees that do not comply risk losing their health benefits. This includes: health, dental, vision, legal, life insurance, supplemental life insurance, ADD, disability, and supplemental disability.
